All federal, state and municipal taxes applicable to this function shall be paid for separately by the patron, in addition to prices herein agreed upon. This price is determined on payment with cash or check. Payments requiring a credit card are surcharged 3% of the total.
***Balance due within 2 weeks after the event billing unless prior arrangements are made. The credit card held on file will be charged with a 3% sur charge if payment is not received 2 weeks of closure of invoice.
IT IS FURTHER AGREED:
- The patron agrees to a 4 hour minimum for each individual staff member that is needed for an event. Time will begin at the time the staff member is scheduled to work at an event unless travel is beyond 60 minutes round trip outside of center city Grand Rapids MI
- The patron agrees to provide all parking necessary for the duration of the event for the staff member(s) requested. If staff have to provide parking for themselves, the cost will be reimbursed by the patron
- All travel required beyond the range will be charged he normal rate of hourly wage at which the individual is working that day.
- All travel necessary outside of 40 miles round trip from center city Grand Rapids MI will be charged at a mileage rate determined by the Federal government standard at that time.
- All travel that requires overnight accommodations will be paid for by the patron as well as all meals necessary during the stay.
- The patron agrees to provide all uniforms necessary outside of the normal uniform that is provided by the staffing agent as listed per the position that is requested.
- Patrons agree to inform the staffing agent at least 1 week in advance as to a definite number guaranteed. Unfilled reservations will be billed.
- All staff acquired through staffing agent are exclusive to the staffing agent. No staff can be hired directly by the patron without written consent of the staffing agent. A fine for $500 or 10% of the first year’s salary (whichever is greater) is subject to claim for breach.
- The staffing agent will exercise all reasonable care in security of the staffing. However the patron will not hold the staffing agent liable if theft, breakage, injury or vandalism should occur, or any other acts beyond care by the staffing agent.
- Patrons agree to receive staff at the event promptly at the scheduled time and to be concluded approximately at the time of departure.
- Patron assumes responsibility for any and all damages caused by any guest, invitee or other person at the event
- In the event of breach of contract by Patron, the staffing agent will keep any deposits and charge for any other breaches to the contract as stated in the contract.
- This contract is contingent upon the absence of accidents or any causes beyond the control of the staffing agent
- This agreement is not assignable.
By sending this contract via email to staffing@culinarycultivations.org the patron is acknowledging that they are putting in a request for services and from that point forward responsible for payment for all requested hours the patron has requested in the staffing form.
